Sega censorship rules were a bit more informal/relaxed then Nintendo during this pre-ESRB era. Sega allowed games to be sold its on 8-bit and 16-bit systems with blood, graphic violence and gore, provided that they had some sort of parental advisory label. Showing or implying sexual issues; nudity, love scenes/sex scenes, French kisses, cross-dressing or homosexuality was still taboo for games made for a Sega system and generally got cut or somehow modified.
Unlike Nintendo, Sega doesn't seem to have anything against hitting girls. Their Streets of Rage series have girls in both playable and enemy character roles.
So according to the american censorship, hitting a whore with a knife is wrong, but hitting a transexual with a cock is right... "In lords name we censor games!!"
The censoring probably had to do with the fact that Sega didn't want to get shit from the soccer moms. Although, the controversy could've been good for Sega if you think about it.
Funny how many games they have you beat up female enemies, but not this one? Guess it was "too realistic"?
Oh and I forgot, the SNES version was only 1 player! It wasn't until FF2 (a SNES only poor ripoff of the first game that was less fun) that you could play two at once! It was an early SNES game...
In the arcade versions, when Roxy or Poison (the female enemies) got punched, their short shirts lifted a little so you could see the lower portion of their breasts. In this version the shirts are longer so it covers them. Their shorts were also shorter.
The blood is removed from sword/knife/bullet/speargun hits.
And finally, the names of some of the characters are changed.
SegaCD US: Thrasher, Katana
Arcade/JapaneseCD version: Damnd, Sodom
As long as I'm posting all the differences, it's also a bit weird, but in the Japanese intro (the SegaCD animated intros are sooo cheesy! hehe) the bad guy has a big animated laugh, but not in the US version. No clue...
Of course the SNES was worse. Not only were the graphics inferior (sure it had a few more colors than this version, but it wasn't arranged like the arcade as precisely), but it was missing an entire level, you could only choose two characters (you either lost Guy or you lost Cody), and Roxy/Poison were turned into these really gay (seriously) looking guys with just differnet colored pants and slightly differnet colored hair. They later made up some crap about them being transvestites.
Final Fight on the PS2 and XBox Capcom Collections is uncensored (well, that is if you don't count the part of the intro where they show Jessica in her bra being held hostage, which was removed).
They are only transvestites in the Super NES and Game Boy Advance versions. Apparently Nintendo of America doesn't like kids beating up 'slut thugs'... haha

The alcohol powerups are replaced with "energy drinks."
The guy says "Oh my car!" instead of "oh my god!" when you trash his car in the bonus round.
The shot of Jessica in her bra was only in the japanese arcade version, and now it shows her in her red dress.
The Japanese MegaCD version of this game is totally uncensored. I have both versions.
