Usually to increase your bench 40 lbs or more you have to gain bodyweight.I would say increase your protein intake.Get a protein powder with 50 grams of protein a serving and take a couple a day.Also don't just bench to increase your bench.Work on exercises like dumbell bench,close grip bench,military presses,dumbell presses and rows for your lats because that's also very important.Also don't max out more than once a month.Maxing more than that will burn u out.
You have potential. Just work on the form a little. What do you weigh?

Isn't the proper form to bench press, you're supposed to bring it down, let it TOUCH, not bounce on your chest, and then press.
