What is the name of the tool you use at 3:42, I can't seem to find it in my default layout.

that's the "insert edge loop" tool... you find it under "edit mesh" ... hold down ctrl+shift and then left click on it to put it onto your shelf :-)

what do you mean? if you're looking for the "insert edge loop" tool make sure you're in the polygons menu.... wich means you have a little dropdown menu in the upper left corner where you can change between the different menus like "animation"... "rendering" ... "dynamics"...etc

i'm trying to do this tutorial and when I import the image can't figure out which perspective should I use. How do I get the image to be flat so I can start modeling?

in that viewpane you want to go to view>image plane> import image
