That's not actually pain coming from your brain, just a sense of pain around your head, the brain has no pain receptors...

The brain can't feel pain, so I think they put you to sleep when they drill through your skull, and then wake you up for the actual surgery.

Its not always as fantastic as this video you need to realise, as far as brain tumors go, the prognosis isn't usually that good. In other words, if you become a brain surgeon, be prepared to Kill 2 See 5 die anyway before you save one.
