During a campaign they speak for hour and hours everyday, 7 days a week with very little sleep. They make mistakes.
The important thing though is that he realized he made a mistake and acknowledged it. Unlike McCain who referred to a crowd as "my fellow prisoners without blinking an eye." And I won't even go into Bush's gaffes...

there is no way, no how he could be using a teleprompter for Q&A. first of all, the set up is standard theater-in-round. he has to constantly address and make eye contact with people on all sides of him. even if you had teleprompters set in every direction, the eye needs to not only line up, it has to refocus, and then pic the exact spot on the text. continued
