The Q is the best siren there is. Because of change in pitch and volume it is the only thing that works with some drivers anymore. It is the only one that should be on a Fire Apparatus.
Sometimes if it a highrise building and the stretcher can't fit through the stairs they can't transfer the patient down from the ladder. I'm not sure if they do that in America but they do that sometimes in Asia and Europe.
Budget cuts, they can't afford to run a ladder and engine so they have a quint. Naprville, IL has 2 stations with just Ladders in them, they run the most Medical calls of all 10 stations.
If you want to see the dutch fireman's, search for "brandweer prio 1" or "politie prio 1" for police and "ambulance a1" for ambulance.

Here in Ionia, Michigan, we run out Ladder for all medicals, or if ladder not in service we run first out engine.
