@Karen12307 I think he mentioned that you don't have to since refractory errors don't count as neurological deficits (at 1:26) - makes sense, I guess.
@Karen12307 I think he mentioned that you don't have to since refractory errors don't count as neurological deficits (at 1:26) - makes sense, I guess.
blind spot test is wrongly done. you have to stare right ahead and not look at top of the pen. look at the top through the corner of the eye and tell when it disappears.
I can't believe people there are people making comments about the patient's physical attractiveness. Absolutely not on.If you can't maintain a professional
objective then at least keep it to yourselves. Pathetic.
Good video. Should really get pt to protrude tongue.XII LMN tongue deviates to side of lesion.Bilat UMN e.g pseudobulbar palsy, spastic, slow moving tongue with dysarthria.(as you know I'm sure)
We were talking about a Cranial Nerve Exam on a normal person not a complete H&P. If it takes an hour and a half to do a CN exam on a normal person you need to shorten it a little. Naturally any problems will take as long as necessary. My understanding was this was a NORMAL NO FINDINGS exam. Maybe 30 sec. was kind of quick but unless there is an equipment malfunction like dead batteries or somebody took the test gear out of the room it shouldn't take over 2 minutes.
I did a full neurrological examination on an ischemic stroke patient 1 month ago... it took me 15 mins in history and almost 20 mins in the physical examination... it's easy and interesting..
I just don't get the blind spot test. He said to her to stare at the end of the pen and as he moves it asks her when it disappears. (I would have thought if you are looking at it directly it wont disappear). Should he have got her to focus on a stationary point straight ahead and then moved the pen asking when the tip disappears and reappears?
@katfrancis If you drive; have you ever moved to the other lane and you didn't see that other car because its next to you but a little bit in the back of you? The blind spot applies here too. (:
Watching this kind of video is strangely relaxing...I think I'll try the shaving and painting ones too, since others have suggested it. Anyway, that aside, good vid.
i think the patient should remove the eyeglasses for visual acuity. for snellen's chart, the physician should start on the letters on the first line and the succeeding letters below should be read by the patient, to assess if the letters seen by the patient is correct
Visual acuity is the patient's best vision period, not necessarily non assisted vision, so if the patient has spectacles or contact lenses, there is no reason for them to remove them.
yes. its the result of the optic nerve coming out at the back of your retina and so no one can see what gets focused onto that spot. But don't you worry, your brain is always smart enough to fill in the visual blanks mentally and so you would never even know you even had one.
should have wiggled both fingers at the same time while assessing the visual fields - tests for visual neglect (this is when patient can see both hands moving individually, but only reports one hand moving when both hands move together)
Got my mock osce tomorrow! So thank you for the quick review!!

this is a very good endevour dr.
but i would like to pinpoint the errors dr. has made while performing the procedure.
1.eyes will be closed while testing for 5th nv.
2.for test of 8th nerve, the tuning fork is to be placed on body of mastoid, not on the tip as shown here.
3.absolute bone conduction test ,an integral part of testing 8th nerve, not done at all!
4.to see palatal movement ,you must use a tongue spatula to depress tongue .
please trectify these.
thanks for a nice demonstration.

Wait till you get in the real world. It's OK to learn more but if you spend more than 30 sec on a CN exam you won't make it in the real world.
