Looks like a GFX Card Issue, your card might be overheating? Or just its time is up. I could be completely wrong but it seems to be the case from the looks of it.
Have you opened it up and made sure all the chips (RAM, video card, etc) are still in solidly? If it's fallen, one of those could've gotten jiggled loose.
