I didn't think the ending seemed very "happy." To me it seems more like he's wallowing in a distant guilt that had become a part of him. The disturbing actions he made (killing his friend) continues to consume or "play/manipulate" his life (hence the fish returning to play music upon his spinal cord). In short, it gave me the impression that death is frightening, but something easy to play with (the Sesame-street-like countdown). I dunno. That's just what I think. =(
