@UserName222222 They use the game clock and the time that the item was picked up. There's an option to display a timestamp when you pick up an item so you can add (or subtract) to the clock as necessary. So if you pick up the mega at 0:03 (or you think your opponent did) then you know it will be back at 0:38.

@UserName222222 Its simply a thought. When human is Nth time in the same situation he exactly knows how to handle it. Its an instinct or habit. You can't consciously count 100 things at the same time - its impossible, but your subconscious can do it easily.
