As far as I know, in MK4 Scorpion did defeat Sub-Zero in battle, but they never said anything about him killing Sub-Zero. That was also the same game when he learned that Quan Chi was the real killer, and there was no mention of the two opposing each other at any point after that.

Only one subzero has died, the original from the first mortal kombat(I can't remember if it's the younger subzero in MK2 or not). Then Subzero turns into Noob Saibot after his soul is tainted by being killed by scorpion.
