The administrative costs of your private plan run about 20 percent, as opposed to Medicare and Medicaid, which run about 10 percent. I admit Medicare and Medicaid don't pay me as much, but they force me to be more efficient with my time.

I've never had a malpractice claim against me, but I know that most of them are settled out of court, by the professional liability insurance company. The facts that establish malpractice are reasonable- it's the awards that are unrealistic.

When my father was in the hospital living on a respirator 4 years ago, we had to make the decision after a few weeks, based on several physicians recommendations to exact his will to end treatment. We were told at the time, this was not really a big decision by one doctor as in Europe, patients are not really given a choice.
