That monologue is so beautiful, and coupled with the song and Ian McKellen's astounding performance I can't help but be moved by this scene every time I see it.
As Seriath said it is true that it's a reprise of "Into the West"..or it's more of a music motif for death carried throughout ROTK. The actual track used in this scene, however, is called "A Far Green Country" and it's on the Return of the King Soundtrack. You can search the title of the track here on YouTube and you should be able to find it.
The reason I love this scene so much is that Gandalf spends most if his Gandalf/Pippin time calling him out on the stupid stuff he does. But here is proof that he does, in fact, care about him as much as Frodo or Sam or Merry. He might be an idiot, but he still marched straight towards Mordor and still charged straight at dozens of Uruk-Hai in defense of Boromir and countless other honorable things.
Gandalf doesn't truly know...he was just comforting the Hobbit in this scene.

Brilliant scene, here. If you are not choked up by this, you must have no soul.
Check out Boyd's neck. You can see his pulse. That, my friend, is Frickin amazing acting.

Gandalf's dialogue is actually from the book, describing Frodo's arrival in the West after leaving the Grey Havens...sorry to be a geek.
Very powerfully used here, though.
