its the idiot in the chevy with his wheels turned the other way.  If you'd ever pulled anyone out in such a situation you would have learned that. it doesn't matter what angle your at on ice when the other truck is off the side of the road. you have to turn the wheels of the truck being towed toward the ditch and it pops right out.

this exact same thing happened with my dakota. big plow came, i went off close to the soulder, hit ice, went into ditch , took a f250 and a ram 2500 together to get me out haha
