The dude's basically torturing the main character's hos to death. In this instance, he's trying to drive a railroad spike into a girl's head, which is a rather painful way to die.

He keeps missing because she's squirming around, and then he hurts himself, gets pissed, and just bashes her skull in with the hammer.
