Trust me, i get these kinds of things happening to me all the time. Halo is so ridiculous. However, this is BUNGIES FAULT. In Halo programming there are visual effects that follow one script and internal effects that control things such as shots and death. There are multiple places on different maps where the bullets will ricochet visually but instead of ricocheting on the secondary kill script, they will continue.

If you can, look back to see if you got any death from the grave medals. If you did, that means that the game SHOWS ou are dead and respawns you, but you actually arent, thats the glitch i heard of. Also, Standbying and lag are also some other factors in this scenario. Very nice video, i cant see any other explinations.
It's Standby! I'm almost 110% Positive. I've standbied before with a few friends as a joke. Also, before i quit Halo 3 and moved to Call of Duty 4, I was a General and witness as well i am guilty of Standbying, && that looks exactly similar. That's my say
If halo is like call of duty the kill cam is not the same as how it is in the real game. Like in call of duty when you are playing and you start sniping and when you look through your scope the scope moves around but in the kill cam your scope doesn't move. Not sure if halo is like call of duty but that's my help.
