hey i had the same problem man heres what you do:

First run the maphack as administrator, made it open but then d2 crashed. If that happens, set the maphack to only do 1 act (instructions at download site) and if THAT doesnt fix the crashing. Run diablo 2 in windowed mode, and that got it working for me.

upload diablo LOD join or make game while in game open your map. leave it open then run the file 'easyload' then it takes like 3 secs then u got it :)

yo if diablo 2 lod doens't work on vista, right click it go to properties, compatability tab, and check the box that says Disable desktop composition. and it should work fine, did for me I have Windows Vista 64-bit

Also, there are a few maphacks that currently work on 1.12, but a lot of them will get you banned. The one that I believe is least detectable is lolwut maphack, as it only uses code injection, not dll injection like most. It's also extremely minimalistic and somewhat buggy, but at least you probably won't get banned for it.
