The notes are mostly accurate and you didn't abuse the BPM column (which is good) but some of the notes are missing. It might be obvious at 1:41 onwards. If you listen carefully, the guitar keeps changing pitch every 1/8 of a measure so you might want to try adding notes there (short long notes are perfect in that area)
It might be incomplete but if I were to rate it, I'd say 7 out of 10. As for the level it should be around 21.
