Obviously he is playing Stepmania. Anyone who plays SM would recognize the GUI and setup. Getting AAA on an easy song like this is nothing in SM. (My point is, don't be impressed thinking he used his feet. He used his fingers on the keyboard for this)

The reason the timing is way off is because he used a computer program to record the screen, but didn't record the sound. He then inserted the song track at a point in the video he thought might be close to when it starts.
