In Season 1 George was scrubbing in with Dr. Burke and he almost killed the patient, Alex (or Christina, can't remember which) started calling him 007, when Mer and Izzie asked why, Alex said it meant "License to Kill." That's why George used it when it couldn't talk, he knew Mer would remember.

There were a lot of reasons why Knight left. A few seasons ago he had some problems with whoever played Burke (forgot his name). And he was upset that his own character didn't get a lot of screen time for Season 5, and that he never really knew what was going to happen next with George. Also, I just want to point out that if George DID NOT die and this WASN'T his funeral (It's not impossible but totally unlikely) unlike Izzie battling Cancer, George got hit by a BUS. It's true, it's George's funeral that you can see in these pictures. The cause of his dismissal isn't quite clear, but it was his decision. So, that said, and after we've seen in the final episode of Season 5, it makes sense.
