I loved this movie. Artistic, violent, graphic, great acting. Everything was pulled off so well. The camera thing was annoying at first, but it grew on me quickly. For the movie Drive (Directed by Nicolas Winding Refn) IMDb states Nicolas Winding Refn sought advice from Gaspar Noé (Director of Irreversible) on how to make the head stomping scene brutal and realistic, but According to Nicolas Winding Refn, the head stomp scene was cut tremendously due to the MPAA stating that it was too violent.

@bubbletang I beleive (if memory serves me right) that Gaspar Noe (director) said that the scene was filmed using a mixture of a dummy and CGI. They tried to use the dummy only but this just looked fake so the CGI was also brought in after the scene was shot.

@webb3ro Correct. It was shot with the real actor, a dummy, CGI, and editing software, which was state of the art for the time, to make it appear as one continuous take\event. I think it's film making at its best. The fire extinguisher was only half as long as the one that appears on screen; CGI filled in the other half that "struck" the victim.

i am not trying to sound all badass here, but after hearing how awful this scene was for a couple years (e.g. one of the most gruesome scenes ever filmed is what i commonly read)... after finally watching it i'm slightly underwhelmed. it's brutal and mean-spirited as hell, but i didn't find it that explicit and didn't feel queasy etc.

I have yet to see this film, but I've heard much about this and the rape scene. And how disturbing they are. Watching this now, and being a big film fan, I think it does seem quite disturbing, not just because of the guy getting his face *slowly* pummled in and cracked with a fire extinguisher, but also the distorted camera, the sound, the odd voices. Ur, the scene seems pretty drunk, and sickening. Really well made. I'm impressed. /No I'm not a 'sicko', just a fan of films that test you!/
