Actually it was not the hydraulics or bearings that lead to this. What happened was a "Chinese Finger" let go of the pipe while the tugger was holding it back. The under roller system could not hold back the weight itself and started to freespool.
The amazing thing about this is not one person onboard the Ocean Intervention was hurt.
