Also I feel really bad for my grandmother, nobody wants to know her now and she had therapy for a while when I was younger but she does not want to be helped because she sees herself as a victim not a persecutor. I stay in contact with her because despite how badly she can behave she is human too and needs someone. I often wish that she would accept help but you can force people into seeking help :(

They tend to be people who call themselves "realists" and dismiss studies on these matters. They ALSO happen to exhibit traits indicative of NPD. It's like a ego defense mechanism. A person with NPD can simply deny the existence of something, and believe it.

Psychotherapy is the least effective treatement. Recent studies have shown Cognitive behavioral treatment most effective. Even just cognitive or behavior show some effectiveness in helping the client or patient to recovery. All personality disorders are more when certain behavior is exaggerated beyond and disregard other people. A times clients or patients harm themselves in thinking the way they think. I'm not saying physically hurting themselves but also may be emotionally or cognitively.
