This actually happend (yes it did) to a woman some years ago that was in front of one of these when it fell of at full speed and pretty much cut her in half when it hit her. Yes she died at the spot.

i haven't seen much about concrete saws, but i would think that the locking bolt can get damaged with continuous repair,or could be made from a cheep steel,or the saw could bind and broke the bolt,or it could be human error.

In most cases, these items are rented from a contractor supply store with the blade attached when delivered or picked up, so perhaps the guy in the footage had nothing to do with the blade being put on to loose. He should of reported it, though.

I think not reporting it was much worse than the sawblade coming off. Blades sometimes work loose. It's bad, but not unheard of.
