  • lmao....hilarious! "S" as in signs and symptoms should have confirmed NOI. Could have gotten a verbal from this pt., that there was no MOI. That said, what purpose did DCAP BTLS serve other than killing time unnecessarily. Don't forget to check condition of pt Nitro., in date doesn't mean it's any good, hot pockets and washing machines are hard on meds.

  • @MsBella121209 Yeah really only one small thing I noticed they didn't do right, at least according to orange book. And it's not a big deal, but you when making initial impression of patient you note the characteristics by reporting color, temp, then whether dry or not. "Skin pale, cool, and clammy"

    In video they did temp & feel first, and color last.

  • I hate when people comment on videos and say " we do this like this " or get too technical for the level of video. THIS IS A EMT B assessment. Therefore chest pain = O2 by nrb 15lpm, and nitro if the patient has it. MED control is alway required before administering medications. Also priroty transport. He did all of the neccessary steps. This vid does'nt have to be "professional" as most EMT's are trained to assess and treat per state requirements. Good for training thats about it. Good job.

  • What about a cardiac monitor? What if this is guy is having an MI? IV? Nitro is not recommended for someone having a right sided MI unless you adequateley increase their preload so you dont bottom out their pressure

  • @santos011 EMT-B cant use a Monitor or IV, Im pretty sure these guys are JUST EMT-B thats prob why, they should of called for ALS

Just remember, algorithms are not set in stone..play with it, this is medical...Patient with chest pain ischemic in origin and a history of it... DCAP-BTLS is not necessary.

    -chest pain=ALS, request the additional resources

    -I recommend asking the date instead of time for A&OX3

    -Guys wearing shades indoors are douchbags

    -o2 is administered in liters, not milliliters

    -don't touch your face during a call

    -this is most likely a medical emergency, worry about checking for dcapbtls en route.

    -write down any info your pt gives you on your run tcket or your gloves. PT's hate repeating themselves.

    -It's cardiac compromise not "heart compromise"

  • (I'm pretty sure) you have to do a quick physical assessment to the patient complain area, and consider a detailed assessment on your way to the hospital...since for test purposes we have to verbalize it only, it just looked different...but you are right detailed assessment is also done on medical. my bad.

  • @Tsamy18 its hard when you're bouncing around in the back of the unit, but totally a time saver. same for BP. granted, cant be done over sweaters, but if its a lighter cloth it'll likely work. try it out sometime!

  • @jo3jo3221 Well it depends on where you are. There are some contraindications for asprin, GI Bleed, Asthma, prego. However where I live you give the patient 324mg asprin (4 baby asprin), followed by the nitro.

  • Why is Aspirin a no no?? I am watching this video cuz...I think I might have had a mild heart attack today or I'm going to have a big one soon. I had pain down my left arm, my chest, my shoulder, nausea, and light headedness....then something squeezing my back and chest til l got down on the ground..to week to stand...then the pain left. I took some aspiran...all day long. is this bad? Please tell...cuz I can't go to a Dr. Yet or he hospital. I'll loose my job. No family to feed my animals.

  • @lmcemt08 The scope of practice for medics and emts in California is so limited that everything is a big no no. As a medic in TN, 12 leads are required in chest pain, Morphine, Oxygen (limited due to current research), Nitro, Aspirin. Hell, we can RSI, as just EMT-P, let alone our CC/EMT-P. CA needs to get with the times.

  • Nice exam, but 5 and a half minutes to get the 1st set of vitals is a bit slow. Also I didn't think there was a need to do a trauma exam. Was ALS called? Good Pt communication.

  • That was good. But you don't reassess SAMPLE and you should have to contact medical control for administration of Nitro. Good assessment though.

  • Medical direction depends on the jurisdiction, for example in MD we can administer three pt perscribed nitros every five mins. with no pain alleiviation, in a prehospital setting as long as the systolic B/P stays above 90 and pul;se stays over 60. Oh yeah if they haev taken cialis viagra or levitra in the past 48 hours we have to perform a consult as well.

  • @breakonthrough27 Depending on your state and their protocols. The first three doses of Nitro is a standing order you do not need to contact Medical Control. The doses include how much the patient took. If at all unsure contact Medical Control.

  • Right now i'm an EMT student (B&I) and this is by far the best demonstration ever lol. I'm diggin' the fist bump and thumbs up. Over here we have to keep in contact with the hospital for almost everything (admin of nitro, etc.) Excellent job

  • in NY its A&Ox3 (person, place, time)...nice assessment, but i dont think your supposed to touch the nitro with a bare hand...(nitro tablet was given to pt. in his hand, in NY we're supposed to give it to them in the cap for the nitro bottle and have them drop it under their tongue via the cap, its their bottle anyway...) Otherwise good...

  • What we were taught in contra Costa county, is that you ask "Person, Place, Time, Event", and if all questions are answered, then the patient is A&Ox4 (Alert and Oriented)
