Months before this happened, Kimberly was with her fiancé Michael when he was driving drunk and crashed, and Michael (and the viewers) were told that she had died.

Kimberly eventually returns to L.A., very much alive. When she and Michael are having sex, she takes a bathroom break, and what you see above happens.

Kimberly's mother brought her to Ohio, where she had brain surgery. As a result, she has gone insane, stealing Jo's baby and blowing up the Melrose Place complex. Good times.
