I hear that in Japan/China, some women get leg surgery to make the legs longer, because they think they'll have a better carreer because of it. That really goes a bit far imo, but those countries are of the extremes anyway.

They don't look like much. Maybe an inch. I've seen tall women and you can always tell the ones who are subconscious about their height because they wear flat, flat shoes all the time and slouch.
