It is based on a novel by 市川拓司. This is a 2003 movie based on the novel, and they made a lot of changes to the original story. The new ただ、君を愛してる is more loyal to the original plot.

its strange... this movie you're saying is exactly the same as "Tada, Kimi wo Aishiteru" also a Japanese movie. And... the main character's name is also Makoto >_> except it's played by Tamaki Hiroshi. And the girl is also named Shizuru (played by Miyazaki Aoi). >_> very strange indeed. The plot is also the same >_>
