If you don't get it, the noise is scalpels being sharpened. Hence the title, "Alternative Therapy". The idea for this song came when Ed and Gavin heard a remix of another of their songs which included the crack of a whip and a girl scream. It was done by a fan for a haunted house and a copy was given to them. They liked it.
