if I remember right, this was from a "Live at Lincoln Center" TV broadcast in the mid-70's. The entire thing was filmed live, so forgive NM any tiny foibles. I remember that in one of the peasant variations she was losing her balance slightly on a pirouette series and the cameraman helpfully zoomed on her upper body to help cover. I don't know if the video that was made later preserved that moment when even she - the ultimate perfectionist - was not perfect!

Great! You just described the ultimate expression of artistry. That's exactly how it felt watching Makarova, there was no evidence of her or anyone in the audience thinking about technique - transcendence is the word that comes to mind.
Without mentioning or acknowledging a certain comment posted here- I can personally attest to a few things, having seen several performances of Makarova's Giselle as a young boy and ballet student at ABT School. I don't recall having ever seen a raised shoulder, or hip. I actually got to watch them Rehearse this in the studio too! Also, High Extensions are inappropriate to the Romantic Style.
I think the stage lighting made her shoulders appear up. The hip is usually released in the Vaganova (in which Makarova was trained)technique. Although I think high extensions are sometimes breathtaking, they are not the be all or end all of technique! High extension does not a ballerina make. Watch Fracci, Alonso or Markova. Then you will see that high extensions are not so important.
