The first time you go up the stairs you'll see a little sister and a big daddy rosi fighting a splicer. Apparently no matter how many little sister you got(3 in this case) you'll always see that fight the first time you go up so the one up there is the extra one because it seems 2k wanted everyone to see that fight despite how many sis u had. To get the extra(4 sisters) make sure to get all 3 little sisters down there before going up the stairs which the 4th always shows but once for the fight.
There is indeed an extra. Get all little sisters down there before going up the stairs and you'll see a rosi and a little sister agasn't a splicer and that's actually the extra one. He was just lucky that the one down there didn't go into the tunnel.
I ran into an extra Big Daddy at this part of the game, but there was no Little Sister along with it. I wish I'd known that before I took the time to kill it... >.<
A tip, don't kill the big daddy until he is finished getting a little sister out of a vent. The big daddy glitched and died in two shots, and the little sister dissapeared.

It's just because the script is programmed to make you see a little sister when you come to this exact place, and it still works even if you have already got the 3 first ones.
@EpicPoop thats how u can do it in that theater place get all sisters FIRST thing u do then 1 will be there due to storyline u have to catch him before he leaves though
