this seems to be cosmesis rather than functional dilemma, but & before plastics touching him, they will need a stable skeletal construct underneath. At 17, his growth potential is by now minimal if ever. He has gross bone loss with complex pseudoarthrosis between proximal ulna and radius. He is lucky not to have developed synostosis. in summary, he needs complex reconstruction with likely need for allograft. u didn't show us his wrist XR. this might affect surgeon's choice. TBC
surgery will be rather major. but its doable. infection must be excluded, and careful planning must ensue depending on general health, and status of his neurovascular, and muscular envelope sanity.
i can show this to my senior elbow reconstruction surgeon, who happens to be the president of British shoulder and elbow society.
Qs will be asked as where is this boy? and can he make it to the UK?
neglected Monteggia fracture victims often have reasonable painless function. when compared with the staged complex reconstruction, patients might risk losing the only thing they had, that is function. they might become painful too. the shortening that is seen in the video of the forearm segment might be not very easy to regain.
if he is still in baghdad. i would suggest that he is seen by an orthopaedic surgeon for at least a primary assessment
