The screamer pops up on two occasions...first i when you are forced to touch her at the beginning of the top of the stairs. the second is when you die too many times (she pops at random moments)

I've played any number of indie games on XBLA, but none have struck me (in a good way) quite like this one. The concept is one I'm not sure I've ever seen in a game, and the atmosphere...

I love this game :) And no, she doesn't jump out at you every time. She jumps out when you pass over her body, and about 10% or so of the time when you die.

I bought this on a whim big mistake! Judge me how you will, but the first screamer only rattled me because you can see her shadow standing there...then I died in the elevators alot and I finally got to the far left elevator shaft and hit the wall, she screamed at me again, and I lost it...Now I can't even replay the first level you see her...
