It's quite unfortunate. I read the entire NTSB report. The first officer who was flying the aircraft was a female with a history of alcohol-related convictions in automobiles and multiple failed or unsatisfactory line checks and evaluations. One line check pilot even called her a "below average pilot" that had issues with good judgment. Another one described performance by stating she had "generally poor airmanship." Why was she given so many chances when she had a history of mediocrity?
Pilot error caused this incident. In this case a woman pilot who as a FedEx employee and any pilot can see in that recreation all she had to do was slightly turn the yoke to the left and hard on the right rudder to keep that plane straight instead of landing sideways like she did.
