No, it can be any level any prestige. The account just needs the perks you want.

You have to pay for each account individually. Generally I just use free trials I receive on the second account.

You can always use a 48-hour trial on a silver account. You also get a free one month of gold when you first sign up an account for live (limit 3 per console).
