Another Twilight Princess death failure that I had:

Before going into the Forest Temple in the beginning of the game, I bought HP potions at a shop, run by a bird. I didn't know how to pay, so I just walked off. The bird complained, then I felt guilty so I went back to pay him. Then he started attacking me. Being hit 12 times without knowing how to pay, he killed me. Game over.

@CIRCLEONE77 its pretty simple in gaming terms, a programmer writes into a program if X then Y. So if X (e.g. mario touchs the side of a mushroom) then Y (mario goes into the air like he's dead and the program restarts the level). Pretty simple really :)
