Woohoo!!!! I figured out how to consistently produce wide screen video with the HQ option enabled.
It's all about the uploaded file resolution 640x360 (16:9 ratio) and the *average* bit rate for video file needs to be over ~2.3 Mbps or so. This yielded a HQ 640x360 flash codec video when viewed on YouTube.
The codec or video length didn't seem to matter in my tests.
I'm not sure if it's purely determined by video bit rate or bits per pixel in image quality. Probably bits per pixel.
Another piece of info I forgot. I uploaded a short 1:15 minute video with starting and and ending solid color (Windows Movie Maker) title credits to see if that effected it. It didn't. It still showed up in HQ. So it appears the very beginning of the video doesn't matter in term of HQ.
If YouTube wants to encourage new-comers (say, channels that are less than 18 months old), then the way to do so is to have a category labeled "New Comers", or "Up Starts" or whatever. And it would even be OK to make this category the default tab in YouTube/browse. This method would make YouTube's intentions clear.
But attempting to encourage new-comers by creating a "Popular" tab that has nothing to do with popularity ("most viewed") is underhanded/sneaky and cowardly.
It is in YouTube's own interests to continue to actively foster the growth of those channels that have already proven themselves by having a wide following. And this active fostering need not conflict with any goal to encourage new channels.
There should be a mechanism for limiting the number of accounts linked to one email - i know a youtuber who has made at least 30 channels and uses them to abuse two or three other youtubers. His comments are hateful but by the time you to to flag him his channel has gone. I sent his name and email to youtube but heard nothing back.

What made Google better than all the other search engine was that they showed the most relevant results up top. They were the only ones that didn't favour results by how much somebody paid to be at the top.
For some reason, with YouTube, they've abandoned what made them the best search engine and they are favouring videos that are paid for over videos that are more popular and more relevant.

I'm glad you revisited the option to manually delete videos from your subscriptions if you didn't want the removing filter enabled. I was actually gonna make a video response about the subscriptions not appearing but now it looks like its been fixed.
I don't think the subscription box on the home page will ever work 100 percent correctly if YouTube tries to rely on it working automatically. Sometimes, a manual solution is more reliable.
1.) If a comment to a video fails to post, you cannot cut and paste your failed comment because the comment box is grayed out and doesn't allow cut and paste. Very annoying.
2.) "Choose my video quality dynamically based on the current connection speed" does not appear to work. I have 5.0+ Mbps DSL connection and it never defaults to the HQ version of a video. It always plays the SD version. My bandwidth is more than enough to play HQ video smoothly.
There is literally no such thing has HQ 16:9 wide screen video on YouTube anymore. It's completely broken. This the quality problem I'm referring to. Upload any high resolution, high video bit rate wide screen video file you wish and see the quality that comes out. It's awful. This isn't just me, the YT "help" forums are filled with knowledgeable people reporting the same thing. Thanks for bringing it up. I wanted to bring this addition piece of information to bear. It's important.

Frankly, I'm against any system that rewards the popular channel any more than they currently are. If popular, one's new video almost always makes the many top 100 most viewed/rated/commented lists. You appear to be complaining that your videos should also be showing up on the "popular" list in addition to these other lists.
The popular list appears to be videos that are videos on channels that are significantly more popular (more views) than the other videos on the same channel.
Views are irrelevant to the most popular list. If you keep you eye on the list, videos with only 1,000 views can often be before videos with 400,000 views. That is why many of us are scratching our heads as to what the algorhythmic formula for landing on the list actually is. The majority of people browsing YouTube only click the 'Videos' tab and don't bother clicking through all the other listings. So Most Popular videos are often the only videos that people see.
This is what I said. The amount of views is _relative_ the number of view the channel has on its other videos. So if a person gets only a few dozen views on an average video and then produces a video that gets a few 1000 views, that's 100 times their average and thus makes the list. I'm sure the selection algorithm is a bit more complicated that this but that's appears to be the basic outline.
This method prevents already popular channels from dominating this list.

Of course people who have worked hard on their channels deserve to be seen, but there are many new people and undiscovered talents who should be seen, as well.
Hi. Actually, what I said was an educated guess by simply looking at the list of popular videos and then looking at the originating channel for each one. Im sure its also calibrated to try to prevent people from trying to game their system by also taking into account the number of ratings and unique comments, etc. Again just a guess but I'm an software engineer so it's my job sometimes to reverse engineer systems so take that for what its worth.

If not, then your insight is quite penetrating. It seems that a lot of small business people are having to spread themselves all over creation to make a few bucks. My primary business is electronics and software development. Now I find myself doing web applications and model aircraft design to make ends meet. Hotforwords may have to do the same thing. So may you.
I'm sure I made some of my suggestions look easier for YouTube to implement than they really are... but most of them just seemed like common sense for a high traffic site.
It seems like YouTube is getting around to addressing some of the issues. Go comment on their latest blog entry about the view counts!

A couple more bugs/problems:

The YouTube HD solution is currently not a good option for a variety of reasons:
1. Requires a relatively new CPU to decode without dropping frames
2. Requires at least 2.3 Mbps bandwidth Internet to stream without annoying stops
3. No HD default user option
4. The SD quality default for HD video is quite poor, needs a middle HQ option as well
YouTube can only fix 2 of these for sure.
(Let alone the file size required to upload)
