I think they mean the usual treatment with mnemonic MONA: morphine, oxygen, nitrites and aspirin. With the goal of keeping muscle death to a minimum.

There are many factors in it :P one of which is that it takes a lot of hard work to become a doctor and generally people from other countries are more hard working because they have incentives.
