It's a MIDI capable foot-synth, made just for this purpose...playing accompany sounds. If you watch older videos of this song, you can watch the progression he makes from NO organ/bass, to using an actual keyboard with pieces of cardboard taped to the keys to play the organ-chord, to finally using the foot-synth.

He had been depressive for 6 years before he committed suicide 1 week ago. His 2yr old daughter died 3 years ago cause of heart disease. He parked his car at a railroad crossing, walked about 100m along the railway before he jumped in front of a train...
