I'm going with the lag scenario, because if it survived transportation and assembly(I'm SHOCKED at this, because I've seen how unstable silver fulminate is), I can't see anything that would have set it off in the air, unless maybe the pebbles were grinding on each other inside the loose wrapper. In that case, the guy's lucky he still has his fingers. My advice is to stick to bird bangers. Bigger pop, more control and much safer.
