So no, it's not strictly speaking a literal hole... it is the remainder of a dead star. Some theories do speculate that the mass is so dense that it tears a hole in space time, but near as far as I know... that's just speculation.
@RomeoCo And even if it does 'tear' a hole in space/time, would it matter? People like to fantasize that we could somehow travel through that hole, but I'm pretty sure we would just be pulled into the mass through gravitational pull and crushed to nothing.

Though the stars are relatively "dense" you're still unlikely to actually hit one due to the still huge distances between them. This is why galaxies can "collide" with no two stars actually hitting one another. Almost all lines drawn through a galaxy don't intersect with a star.

This works really well, if you're going to do pop astrology then do it 100%
Although you wouldn't see the black hole unless you passed through the event horizon otherwise all you'd see is a concentration of shimmering light.
