The default thumb for a video is the image in the middle of the video. He put pop up boobs in the middle to make the video appear to have sexual content (that image used to be the thumbnail for this video). As Kasem pointed out a lot of people do this on you tube and it is misleading. Like he said in the comment, he changed the thumb, as it was annoying him.
