Ok, I'll break it down for you.
The figures you see in the video (the chick with the swords, etc) are the four horsemen. War (sword chick) Famine (skinny dude against wall hardly moving) Pestilence (dancing chick with the "boils" on her face) and Death (the guy in the funeral suit with his face painted like a skull).
The numerology in the video comes in sets of 3's and 7's. So what is being shown is 3 7's or 777 which is "Gods Number" or the number of completion.

I think Jay-Z just wanted to make a controversial video to get people talking. Everybody seems to loves finding illuminati or occult references in stuff these days so why not go for media attention that way! And judging by the amount of videos about his music, and in particular this song, id say he is succeeding at getting his name out there!! As for the 3 lines = 666 . . . talk about clutching at straws!
