There's no exaggeration here, fame and blood do go together. The concept behind Gaga's performance is that fame can be fatal. Celebrities worship Fame like a religion and eventually it kills them, whether that comes about from the drug culture of the music industry, suicide, or in the case of Princess Diana, the paparazzi themselves. Gaga's talked about all of this in interviews even prior to her VMA performance.
