idk if anyone else knows this but Friezas finale form is when he fought Goku. and that was only his 3ed form. When Goku fought Cooler in the DBZ movie Cooler found a 4th form. When you first see Frieza that is his normal look. So there is no 5th form that I know of.

There's a difference between a form and transformed state, I wish his dialogue was a bit different because people get confused when he says that. His 1st form isn't a transformed state because he doesn't transform to get to it. 1st form to 2nd form=1 transformation. 2nd form to 3rd form= 2 transformations and 3rd for to 4th form = 3 transformations.
