Do you have to do the Hidden Race or can you progress normally through the game?

to do the insurgent run you must do the hidden race, if not you'll be forced to get more than 4% of the items.

there are a couple of requirements that i mention in annotations, but you can't touch the door (touch or shoot with grenades) at 5:22 and you have to get to that room in 30 seconds from the three barrels at 5:39. My guess is you are either accidentally touching that door or not going fast enough from those barrels.

Wait... picking up that health upgrade... wouldn't that ruin the 4% aspect of this run? I thought you weren't allowed to pick up anything other than any gun, grenades, climbing gear, the flashlight, foam, and missiles.

You can pick up one item if you left the Pistol (not sure if the Pistol counts towards that list of items described in the challenge now that i think about it....) and only get 4%. If you watch the stats screen in the last video you will see that i do in fact get the Insurgent award.

Yeah. I just did did it with 4% today, (with gold pistol, because I wanted a challenge :P ) and I did that picking up the extra health... dunno how I could have beat the game without that. And later on in the game, I accidentally picked up the AR6, (I had foam, missiles, climbing gear, flashlight, and grenades at the time) which bumped my % from 4 to 5, so I'm guessing grabbing the regular pistol at the beginning DOES count towards your percentage the same way.

actually, i'm wondering if the pistol counts towards that list of items as another item (flashlight, foam, etc.) i knew it counted towards the 4%

Well, I'm not sure exactly if the pistol does, but I know the AR6 did make my % pass from 4 to 5 after I had gotten all that I needed plus the extra health pack. So maybe non-golden weapons count towards your %, but gold weapons don't.
