It's easier than you think. Since you drive with such a limited field of vision, it's hard to catch a drop in the terrain sometimes, especialy if it inclines right up to the ledge. Believe it or not, it happens quite a bit.

Driver-"that looks awefully steep" / Tank Commander - "we'll be fine, go" / Driver - "are you sure?" / TC - "Jesus just go, GO!" .....
