She's "fat" in this episode because her character has a weight problem (she's wearing a body suit and face makeup to make her look heavier). To lose weight, she starts drinking only green smoothies, but the greens having been growing near some of the special meteor rocks that came down when Clark arrived in Smallville as a child.

The meteor rocks gave the greens she's eating the power to make her lose weight VERY quickly, but her body also craves the nutrients she's lost from losing 20 pounds in only a few minutes. I love Smallvile, but man...this episode, and most of the FOTW episodes from the first two seasons...just awful. What's really sad about this episode is that they didn't even make her look fat. They just added some baby fat to her cheeks. Maybe her perceiving herself as worse off than she was was part of the point, or maybe it was a commentary on society. I'm just glad Smallville grew up over the years and isn't this same old same old formula. :)
