I think the ox peckers are helping the Giraffe by preventing maggots and ticks from entering his body. I don't believe they are making the wound any worse. Animals that welcome these birds survive at a better rate then animals that don't which is why many animals let this pekcers even peck their eyes...simple darwiniasm

Can you see the muscular contractions? The giraffe is trying to scatter away the flys and a open wound is a paradise for larvs. Most likely that the birds aren't eating the flesh, but feeding on fly larvs instead.
