Foley/Mankind had already been thrown from the top of the cell & then fallen through the top of the cell, with the chair falling after him & hitting him. I think this was when Undertaker threw himself at Mankind, but Mankind crumpled to the floor out of reach & sight so Undertaker looks like he's diving at the cage randomly.
@jyawsome I've only ever seen "Chucky" 1 & 2 & it wasn't in them, but there's so many others that I haven't watched, so I hope that somebody else can answer this. : )
@Mikill7 I haven't heard any reports about an injury caused by this particular dive, but I'm sure that it all adds to the physical problems that he suffers from.
